The Chronic Disease Fund (CDF) can help you if you have MDS (or another covered disease) and you have private insurance or a Medicare Part-D plan, but cannot afford your co-payments for your medication. If you meet their eligibility requirements, CDF will pay some of your out-of-pocket costs for medication. CDF can also provide counseling to help you better follow your doctors' instructions. You must fill out an application that will determine if you are eligible. If CDF cannot help you, they will refer you to other resources for assistance.
